I’m currently editing a form view where I have the display name for each column. However, whenever I add a description for that column, it instead shows the description and hide the display name. Any idea how do I fix this? Any suggestion is greatly appreciated

OK, then it’s a normal behavior.
In Form views, Description content is displayed instead of Display name. It is intended to provide additional context to users when filling a new information.
You may want to get rid of the Description part is you wish the display name to be displayed in forms.
Wow I didn’t know that. I guess it isn’t really convenient, since I want to have the description as a smaller text under the display name so that users know what to fill in each field. Do you know if that’s possible? If not then thanks for helping me with the issue!

You may want to use a type show column to add context or additional information, but what you want to perform is a little complex to set.
The more simple way would be to use for example:
Display name: “Name”
Description: “Name: your family name”
