what did i do wrong??? 
You probably created a slice without the adds allowed setting. Or the table setup in with Adds unmarked.
The 3rd possibility is that the add action in behavior is marked and do not display on Appearance setup

Table setup you can check on the left menu: DATA - TABLE - TABLE SETTINGS on the top right side
Slice setup - DATA - TABLE - SLICE - Slice Actions - ADD action must be on it
3rd option: Behavior menu - find the the table that you this action is - expand the add action menu - Appearance - Display as primary

no im lost …
it seems that is a detail view. Put the action apparence option on display inline and choose the NOTE column to attach it
