How can I create a Google form with patients first name , last name, DOB, Male/Female, Zip code, Date of Test. When they hit submit it will show them the result as Positive or negative which is already filled in Google Spreadsheet. Is this possible?
I want to show the result only when above 6 conditions are met.

Seems like a simple problem and it can be done for sure, you have to store that same data on the table with the results and then when they submit the same data you can have a column/field that checks if for that string is a recent result and what the result is
