Please can anyone share a template app for completing risk assessments at all? I am looking to pull together solution for completing site risk assessments.

I don’t have a template I can share, but I can show you one of my workflows for inspiration:
Normally the lat/long wouldn’t drop you in the ocean, but I had location disabled for the video.

Which type did you use for making this risk matrix at second 30’’ of your video ?
We tried to reproduce it with LongText/HTML, it does not give the expected result as CSS are ignored 
Did you do it with Images that switch depending on criteria values ?
Top: ours
Bottom: yours
