I am creating a form for our app users to reach out with any questions. I am looking to have it do the following:

1st page will be which type of contact method with options to choose either phone or form.

  1. If phone is selected it will go to another page in the form showing our number to call.

  2. If the form option is selected then it will show fields to fill out such as name, title, organization etc. with a save option that will send us an email.

What is the best way to accomplish this as well as the app formulas, types etc.?

@John_Beck How about using 2 actions: Phone action will start a phone call. And form action will go to another view (using a LINKTOFORM formula). You can use a workflow to get an email whenever a new form is saved.
