I have an app I created for inner company use and it’s showing some text at the top of the form that I have no idea where it came from. Any help with figuring out how to edit or delete this text would be greatly appreciated!

That would be either the display name or the description.
If you only see this in the form, check the description text.

the description you are referring to would be the description of the form itself?
The description text for that datetime field.
